Getting your first aligners? What happens at your fitting appointment

Key points

  • Before you leave, we check that each aligner fits your teeth properly.
  • Some teeth may have small tooth-coloured “attachments” bonded to them. These help the aligners grip and move the teeth as planned, and they are removed at the end of treatment.
  • Some treatment plans include IPR (interproximal reduction), where a very small amount of enamel is polished from between certain teeth to make space. It is only done if it is part of your plan.
  • We show you how to put your aligners in and take them out, and how to clean them.
  • Aligners need to be worn for most of the day and night, usually 20 to 22 hours a day, or as your orthodontist advises. Take them out to eat, to drink anything other than plain water, and to brush.
  • It is common for new aligners to feel tight for the first few days. You will move on to each new set at the interval your orthodontist sets, with review appointments along the way.

Summary

Your first aligner appointment is mainly about checking fit and making sure you feel confident looking after your aligners at home. In this short video, Dr Sarah Sadek explains what happens step by step, from checking each aligner and placing any attachments, to how long to wear them and what to expect in the first few days.

Clear aligners only work when they are worn as prescribed, so the appointment also covers how to fit them into daily life: taking them out for meals, cleaning them, and keeping them in their case when they are out. After treatment, retainers are needed to help keep teeth in their new position.

Consultations and fitting appointments take place at Waterloo Dental & Orthodontics on Waterloo Road, a short walk from Waterloo station.

Questions from this video

Do new aligners hurt?

Most people feel some pressure or tightness for the first few days of each new set. This is normal and usually settles. If an edge feels sharp, or discomfort does not settle, contact the practice so we can check it.

How many hours a day do I need to wear aligners?

Usually 20 to 22 hours a day, taking them out only to eat, drink anything other than plain water, and clean your teeth. Your orthodontist will tell you what your own treatment needs.

What are aligner attachments, and are they permanent?

Attachments are small tooth-coloured bumps bonded to some teeth so the aligners can grip and move them. They are not permanent: they are removed at the end of treatment and the tooth surface is polished.

What is IPR?

Interproximal reduction (IPR) means polishing a very small amount of enamel from between certain teeth to create space. It is not needed for everyone and is only done if it is part of your treatment plan.
